Going Home

Your attending physician will tell you in advance when you may be discharged from the hospital. Please be sure to check your room carefully for any personal items you have brought with you and to collect any valuables placed in the safe.

Your doctor will prepare special discharge instructions for your nurse to review with you and/or a family member prior to going home. This is an appropriate time for you to ask any questions concerning your recuperation. All family members and caregivers are invited to attend a group meeting on caregiver advice offered at 4 PM every Thursday in the fourth-floor conference room.

Upon discharge, you will be escorted to the main entrance on Delaware Avenue (main lobby) at the canopy drive-through entrance. The person providing your transportation may leave their car under the canopy briefly while they are picking you up.

Home Care

Should you require additional nursing care or service after you are discharged from the hospital, or if you need support from medical equipment in your home, we can arrange for these home healthcare services on order from your physician. Consult with your Patient Care Manager to select the provider of the home health service you require.

Fayette Home Care and Hospice is the preferred provider of home health service for Uniontown Hospital. The services available to you at home include skilled nursing care, enterostomal therapy, intravenous therapy, enteral and parenteral nutrition, laboratory services, physical, occupational and speech therapy, medical social work service, home health aides, palliative care and hospice services.

The medical equipment and patient-care supplies available include hospital beds, suction system, oxygen delivery system, bathroom safety aids, walking aids, wheel chairs, self-diagnostic equipment, and an array of accessories.
