Hospital partners with Aetna

10/20/2015

Uniontown, Pa - October 20, 2015 — Aetna (NYSE: AET) today announced a new Medicare Advantage plan with a low monthly premium and robust benefits for Medicare beneficiaries in Fayette County. The Advantra Fayette Health Prime Plan is a Medicare Advantage HMO based on Uniontown Hospital’s provider network, its affiliated physicians and other health care providers.

Plan highlights include a $5 copay for primary care physician office visits, a no-cost fitness membership to multiple gyms and more.

Medicare Advantage members enrolled in the plan who receive care through the Uniontown Hospital’s network and affiliated providers will experience more coordinated care. With the new plan, Aetna and treating physicians can better share information, which is particularly beneficial for members with chronic or complex conditions. Aetna nurse case managers will work with providers to assist in care coordination, outreach and follow-up services.

“We are pleased to offer Medicare beneficiaries a new low-premium plan that focuses on collaboration and quality of care,” said Laurie Brubaker, market president for Aetna. “Uniontown Hospital doctors and other health care professionals will work closely with us to help promote wellness, provide quality care for chronic conditions and use economic incentives for providers to reward positive health outcomes.”

“Our patients are the primary focus and concern of our staff each and every day,” said Steven Handy, CEO of Uniontown Hospital. “With that in mind, this new partnership with Aetna will allow area residents a meaningful alternative for health care coverage and also will enhance our ability to coordinate care, which is why we are so pleased to be a part of this progressive collaboration. This will be another way we can make a healthy difference for Fayette County.”

This plan will be available in Fayette County during Medicare Annual Enrollment period this fall with an effective date of January 1, 2016.

Aetna has been working with health care systems to advance value-based models of care for more than seven years. In value-based models, Aetna pays doctors and hospitals for helping keep people healthy and for improving the health of those who have chronic conditions in an evidence-based, cost-effective way.
